Community Connectors aim to improve health and wellbeing outcomes for older people in our community using person centred approaches to support resilience, independence and improved access to community supports.

Job Description

As a Community Connector you will work in close partnership with the Queens Cross Retirement Housing & Intervention team. You will work alongside local and citywide organisations to strengthen information links between the Third Sector, older people, carers and statutory services.   


In addition to improving access to information about community based support and activities for older people, you will offer a person-centred service that is responsive to the needs and interests of local older people and their carers. You will support older people to identify issues that affect their ability to live well and work collaboratively with them to support them to make decisions on the issues they would like to address.  If appropriate, you may then help them to access services that best meet their needs.


Using person centred approaches you will work with people from a wide range of backgrounds who may have complex social and emotional issues. A strong understanding of the challenges faced by older people and their carers and an ability to build rapport with people are all essential to this role.


You will be expected to develop good working relationship with Queens Cross Retirement Housing & Intervention team, referring partners and have the ability to foster links with Third Sector organisations and statutory agencies to improve connections and develop pathways at a local level.
